You may review and edit this information before submitting. After the page loads, the website is designed to remove your name and email address from the visible browser URL.

When you complete the survey, we may collect:

• State or general location
• Your connection to end-of-life work
• Survey selections and optional written responses
• Whether you request newsletter access
• Whether you enter a promotional drawing

Please do not include confidential client, patient, medical, or other highly sensitive personal information in written survey responses.

Our hosting and technical providers may also receive limited technical information such as IP address, browser type, pages visited, referral information, and security or error logs.

We may use information to:

• Administer and analyze the survey
• Understand trends and needs within the end-of-life field
• Improve educational resources and professional support
• Prevent duplicate or abusive submissions
• Administer promotional drawings in accordance with the applicable Official Drawing Rules
• Contact drawing winners
• Deliver newsletter and training resources when separately requested

Survey findings may be used or shared in aggregated or de-identified form. We will not publicly identify individual respondents without permission unless required by law.

Participation in the survey is voluntary.

Before beginning, you may be asked to confirm that you reviewed this Privacy Policy and agree to participate. Survey participation does not automatically subscribe you to any newsletter or marketing list.

After completing the survey, you may have the option to request complimentary one-year access to a curated end-of-life newsletter, valued at $200, along with selected educational or training resources.

Newsletter enrollment is separate from survey participation and requires an affirmative opt-in.

If you opt in, ongoing communications may be sent by Well Supported Death from the wellsupporteddeath.com domain, even though the survey itself was hosted or promoted through awellsupporteddeath.com.

The newsletter offer:

• Does not create a paid membership
• Does not create an automatic paid renewal
• Does not require payment information
• Does not authorize any charge
• Can be canceled at any time through the unsubscribe link in each email

You may also receive educational emails related to the resources or training access you requested.

You may have the option to enter a monthly drawing connected with the survey.

No purchase or newsletter subscription is required. Newsletter subscription does not provide an additional entry or improve your chances of winning.

If you enter, we may use your name and email address to administer the drawing, verify eligibility, select a winner, and contact you if selected.

If there is any conflict between this Privacy Policy and the Official Drawing Rules regarding administration of the drawing, the Official Drawing Rules will control with respect to the drawing.

We may use Mailchimp to store contact information and survey responses, manage subscription status, organize audience data, and deliver newsletter communications.

We may also use third-party service providers for:

• Cold-email outreach
• Email delivery
• Contact management
• Security
• Technical maintenance

These providers may process information on our behalf as needed to provide their services.
